Malaria parasite test (smear and RDT)

What it measures
Detects Plasmodium parasites in blood. Thick and thin Giemsa smears identify species and parasite density; rapid tests detect HRP2 or pLDH antigens.
Specimen and preparation
- Specimen: EDTA whole blood or fingerprick
- Tube: Lavender top
- Preparation: Collect during or soon after a fever spike; repeat every 12–24 h up to 3 times if negative.

Reference ranges differ between laboratories, methods and populations. Always use the range printed on your own lab report.
Causes of high values
- P. falciparum (high parasitemia is severe malaria)
- P. vivax, P. ovale, P. malariae, P. knowlesi
- HRP2 may stay positive weeks after cure

Giemsa-stained thick/thin films; immunochromatographic RDT; PCR
Good to know
A positive malaria result is a critical value. RDTs miss low parasitemia and pfhrp2-deleted strains; microscopy remains the reference.
