COVID-19 test (RT-PCR / rapid antigen)

What it measures
Detects SARS-CoV-2 RNA by PCR or viral proteins by rapid antigen test in respiratory samples. PCR is more sensitive; antigen tests give quick results when viral load is high.
Specimen and preparation
- Specimen: Nasopharyngeal or nasal swab in viral transport medium
- Tube: Swab top
- Preparation: Collect within the first days of symptoms; keep sample cold.

Reference ranges differ between laboratories, methods and populations. Always use the range printed on your own lab report.
Causes of high values
- Current SARS-CoV-2 infection
- PCR may stay positive for weeks after recovery (low viral load, high Ct)
- Antigen false positive (rare; confirm in low-prevalence settings)

Real-time RT-PCR; lateral-flow or fluorescent antigen immunoassay
Good to know
A negative antigen test in a symptomatic person should be repeated or confirmed with PCR.
