Neutrophils
Hematology · also called Polymorphs, Segs, ANC, Absolute neutrophil count
What it measures
The most common white cell and the first line of defense against bacteria and fungi. Reported as a percentage and an absolute count; the absolute count guides infection risk.
Specimen and preparation
- Specimen: Whole blood (EDTA)
- Tube: Lavender top
Reference range
| Group | Range |
|---|---|
| Adult (%) | 40–75 % |
| Adult (absolute) | 2.0–7.5 ×10⁹/L |
Reference ranges differ between laboratories, methods and populations. Always use the range printed on your own lab report.
Causes of high values
- Bacterial infection
- Acute inflammation or tissue damage
- Corticosteroids, stress, exercise
- Chronic myeloid leukemia
- Pregnancy
Causes of low values
- Chemotherapy and certain drugs
- Viral infection
- Benign ethnic neutropenia
- Aplastic anemia or marrow infiltration
- Overwhelming sepsis
Method
Automated differential (flow cytometry/impedance) with manual smear review when flagged
Good to know
ANC below 0.5 ×10⁹/L is severe neutropenia with high infection risk. Toxic granulation and left shift on the smear support infection.
