What it measures
Large phagocytic white cells that become tissue macrophages. Raised counts often point to chronic infection, inflammation or certain leukemias.
Specimen and preparation
- Specimen: Whole blood (EDTA)
- Tube: Lavender top

Reference ranges differ between laboratories, methods and populations. Always use the range printed on your own lab report.
Causes of high values
- Chronic infections (tuberculosis, endocarditis)
- Recovery phase after neutropenia
- Inflammatory bowel disease and autoimmune disease
- Chronic myelomonocytic leukemia
Causes of low values
- Hairy cell leukemia
- Aplastic anemia
- Corticosteroid therapy
Method
Automated differential with smear review
Good to know
A persistent monocyte count above 1.0 ×10⁹/L in an older adult warrants a smear review for CMML.
