Malaria thick and thin films

Principle

• Thick films concentrate many layers of blood; red cells are lysed during water-based staining, increasing sensitivity for detecting parasites.
• Thin films are fixed so red cells remain intact, allowing species identification and parasite density as % infected RBCs.

Specimen

• Fingerprick blood, or EDTA blood with films made promptly (ideally within 1 h) to avoid morphological changes.
• If negative and malaria is suspected, repeat every 12–24 h for three sets.

Procedure

• Thick film: spread a small drop (a few µL) into a circle about 1–1.5 cm across; newsprint should be just readable through it. Dry thoroughly; do not fix.
• Thin film: standard wedge film; fix in methanol after drying.
• Giemsa stain buffered to pH 7.2: about 3% for 30–45 min or 10% for about 10 min. Field stain is a rapid alternative.

Results

• Thick film is declared negative only after examining at least 100 oil-immersion fields (many labs use 200 or more).
• Parasite density (thick): parasites/µL = parasites counted ÷ WBC counted × patient WBC/µL (use 8000/µL if the count is unknown). Count against 200 WBCs, or 500 if fewer than 10 parasites.
• Thin film: parasitaemia % = infected RBCs ÷ total RBCs × 100.

Interpretation

• P. falciparum: normal-sized RBCs, small delicate rings, double chromatin dots, multiple infected cells, appliqué forms, banana-shaped gametocytes; schizonts rarely seen in blood.
• P. vivax: enlarged RBCs with Schüffner's dots, amoeboid trophozoites, schizonts with 12–24 merozoites.
• P. ovale: enlarged oval cells, sometimes fimbriated, James's (Schüffner-like) dots.
• P. malariae: normal or small RBCs, band forms, rosette schizonts with 6–12 merozoites.
• P. knowlesi: early forms resemble falciparum, later forms resemble malariae; confirm by PCR.

Quality control and pitfalls

• Platelets over RBCs, stain precipitate, Howell–Jolly bodies and debris mimic parasites.
• Wrong buffer pH hides Schüffner's dots and alters colours.

Clinical use

• Gold standard for malaria diagnosis, species identification and monitoring treatment response.
