Faecal elastase-1

What it measures
Faecal elastase-1 is a pancreatic enzyme that passes through the gut largely intact. Its level in stool reflects exocrine pancreatic function and screens for pancreatic insufficiency.
Specimen and preparation
- Specimen: Formed stool sample (about 1 g)
- Tube: Stool top
- Preparation: Watery stool dilutes the result; pancreatic enzyme supplements do not interfere.

| Mild–moderate insufficiency | 100–200 µg/g stool |
| Severe insufficiency | <100 µg/g stool |
Reference ranges differ between laboratories, methods and populations. Always use the range printed on your own lab report.
Causes of low values
- Chronic pancreatitis
- Cystic fibrosis
- Pancreatic cancer or resection
- Diabetes (some patients)
- Watery diarrhoea (dilution)
