ABO and Rh(D) blood grouping

What it measures
Identifies the A and B antigens on red cells and the matching antibodies in plasma, and tests for the RhD antigen. Required before transfusion, in pregnancy and for donors.
Specimen and preparation
- Specimen: Whole blood (EDTA)
- Tube: Pink top
- Preparation: Label the sample at the bedside with two patient identifiers; many labs require a second sample to confirm group before issuing blood.

Reference ranges differ between laboratories, methods and populations. Always use the range printed on your own lab report.

Forward (cell) and reverse (serum) grouping by tube, slide, gel column or microplate agglutination; anti-D typing
Good to know
Forward and reverse results must agree; discrepancies occur in newborns, elderly patients, weak subgroups, cold antibodies and after stem cell transplant. Weak D may need extra testing.
